림프절, 특히 mediastinal lymph node aspiration에서 가장 주의해야할 병변이 small cell carcinoma와 precursor lymphoblastic lymphoma (또는 T-ALL) 과의 감별입니다. 게포가 거의 유사하게 보일 때가 있고, 임상적으로도 lung mass가 작거나 가려진 경우 감별이 어렵습니다. Organoid pattern 여부가 가장 중요합니다.

매우 homogenous smear로서, 핵의 크기가 small lymphocytes의 1.5배 정도 (2배까지는 되지 않음) 되는 intermediate size입니다. Chromatin이 아주 섬세하고 salt and pepper pattern을 보입니다. 이런 형태의 lymphoma는 lymphoblastic lymphoma 또는 Burkitt lymphoma가 대표적입니다.

비교적 균등한 크기와 모양을 가진 세포들로서 organoid pattern이 없이 singly scattered pattern이며 histiocytes도 종종 발견됩니다. 정상 small lymphocytes의 핵에 비해 종양세포 핵의 크기가 1.5-2배인 intermediate cells로 구성되어 있고, mitosis가 흔히 관찰됩니다. 핵소체가 뚜렷하지 않고 fine nuclear chromatin pattern을 보입니다. 액상 도말에서 Burkitt lymphoma까지 진단하기는 어려운 점이 있습니다.

비교적 균등한 크기와 모양을 가진 세포들로서 organoid pattern이 없이 singly scattered pattern입니다. 정상 small lymphocytes의 핵에 비해 종양세포 핵의 크기가 1.5-2배인 intermediate cells로 구성되어 있고, apoptotic bodies가 많습니다. 처음부터 세포학적 진단으로 Burkitt lymphoma로 진단하기는 어려우나 malignant lymphoma, high grade 라고 할 수 있습니다.

Internal control로 보존이 잘 된 가장 작은 세포:small lymphocytes와 비교하여, 핵의 크기가 2-3배 가량 커진 large lymphoid cells가 많이 증가되어 있습니다. 정상에서도 Large lymphoid cells가 도말되긴 하지만, 한 high power field에 두섬두섬 (5-10개) 보이는 것과 달리 high grade lymphoma는 sheet처럼 등장하기도 하고 매우 많이 증가되어 있습니다.

Large lymphoid cells가 많고 apoptotic bodies도 많아 atypical lymphoid lesion입니다. 환자의 나이가 10살인 점과 임상적으로 인후통과 함께 발열, liver function test 이상 등의 소견이 중요합니다. Kikuchi lymphadenitis와는 달리 apoptotic bodies는 많으나 cresecentic nucleus를 보이는 phagocytic histiocytes가 거의 없습니다. Malignant lymphoma와는 매우 혼동되기 쉬운 병변입니다.
